The Chapter Policy Review is Due October 15th, to fill out this form follow the below instructions. 

Step1: Hover over the FORMS tab
Step 2: Hover over COLLEGIATE CHAPTER REPORTS
Step 3: Go down to the fourth form CHAPTER POLICY REVIEW 
Step 4: Click on the form and fill out
